Home at Last

The Home at Last (HAL) provides day-of-discharge help to aging or frail adults or adults with disabilities to ensure their safe and comfortable return to independent living at home after hospital discharge. 

Our staff provide transportation from the hospital, perform errands such as prescription and grocery pick up, as well as provide a complimentary meal. Program staff will follow up at a later date to make sure you or your loved one is staying safe at home.  

We can also help you or your loved one find resources available in your community to support ongoing safety and independence following hospital discharge.

We can help with picking up a prescription or cooking a meal, simple tasks made more difficult after returning from hospital.

We work collaboratively with the hospital and other community support services to ease your transition from hospital.
