Family Supports: Passport Program

The Passport program supports community involvement and independent living for adults with developmental disabilities.

We work with adults who receive Passport funding from the Ontario government and their families. We help arrange services eligible for the funding, including personalized support plans, finding support workers, arranging caregiver respite and more.

Passport funding is available from the Ministry of Community and Social Services, through Developmental Services Ontario. Adults with developmental disabilities 18 years and older are eligible to apply. 

Funding is to be used for activities of daily living and caregiver respite. Once you receive funding, we can assist you to develop a personalized plan to help you achieve your goals. We can assist you with finding the right support worker and provide referrals to other sources of community support. We can help arrange respite for family caregivers.
